See Firmino says his dentist helped him launch his career and has been very good to him. That makes so much sense.

In a culture where large areas are poverty stricken or just on the breadline community servants, and especially highly educated ones like doctors, dentists and school teachers, often become something of a community leader. I don't think Firmino exactly grew up in a favela but it's very possibly that the local dentist was a 'big man' of the neighbourhood and very involved in community projects and affairs.
